As a new patient, you can contact our main office (713) 795-5511 or north office (713) 691-0107 for an appointment. Your health may require that you have a referral from your primary care physician before you can be seen and treated. If so, please provide referring physician contact information at the time of your appointment. We will also need access to recent laboratory and imaging studies. All new patients are required to provide insurance information at the time of scheduling.
Its easy. Once you contact our office, you will receive two emails, one inviting you to join the portal, MYKIDNEYASSOCIATES.COM and the second enabling you to fill out your new patient forms online. The invitation will include instructions. This is a quick process. We will need your first name, last name, date of birth, zip code and e-mail address. To verify your identity, your information must match what we have in our system.

If you cannot do this at home, no problem. We understand, and will help you fill these forms out in the office, plus get your consent and HIPAA paperwork completed. We want to make your experience as a patient as comfortable as possible.
